NorthCoast Health Care Management Services is an integrated network of providers for home health, home infusion, and hospice services. We provide comprehensive management of the patient’s health plan benefits for appropriate utilization of services for the patient and the health plan. Our goal is to obtain the highest patient outcomes in the most cost efficient manner possible.
NorthCoast’s extensive panel of providers, all credentialed and contracted, provides services throughout the state of Ohio and the contiguous counties from the surrounding states. With over 250 providers, our network is comprised of hospital based, not for profit, and independent providers. They are local, regional and national providers dedicated to providing the highest quality care in the home and other alternate site settings. Our network provides services for Wellpoint/Anthem, all Blue Cross/Blue Shield plans, and Medical Mutual of Ohio.
